He will be joined by Conor O'Shea, a former Irish rugby international who is now director of rugby at English Premier League champions Harlequins.

O'Shea has been appointed because of his background as former national director of the English Institute of Sport and his expertise and understanding of coaching as well as sports science and medicine. 

Lurz, who won the Olympic silver medal in the 10 kilometres open water swimming at Beijing 2008 and bronze at London 2012, will bring an international perspective to the review.

The panel will be chaired by Craig Hunter, an independent member of the British Swimming Board and the current Chef de Mission of Britain's Paralympic team competing at London 2012.

It will be completed by the sport's performance director Michael Scott.

Britain finished 15th overall in London with only three medals, a silver and two bronzes – well below the minimum target of five they had been set.

The poor performance has put in jeopardy the amount of funding the sport receives from Government agency UK Sport, who in its last round of funding in 2009 awarded it more than £25 million ($39 million/€32 million) for the next four years.

"The aim of the debrief is to consider the performance results at the London 2012 Olympics, to determine the reasons for below medal target performance and identify key areas for improvement for inclusion in British Swimming's strategy for Rio and beyond," said a British Swimming statement.

The review is due to be completed before the end of October.
